Tammy Perkins, 43, Injured in 18-Wheeler Crash in Texas

Pollok, Texas  (January 31, 2026) — A Saturday afternoon crash on U.S. Highway 69 sent a Pollok woman to the hospital after her vehicle collided with an 18-wheeler at a rural intersection, according to the Texas Department of Public Safety. Tammy Perkins, 43, sustained non-life-threatening injuries in the two-vehicle collision, which occurred near FM 2021.

Incident Details: The crash occurred at approximately 4:17 p.m. on U.S. Highway 69 at FM 2021. According to the preliminary crash investigation, a 2000 Freightliner truck tractor semi-trailer was traveling northbound on Highway 69 when it slowed to make a right turn onto FM 2021.

Perkins was driving a 2025 Kia K5 northbound behind the semi-truck. Investigators said Perkins failed to control her speed and struck the rear of the semi-trailer as it slowed for the turn. Emergency responders arrived at the scene and transported Perkins to St. Luke’s Health–Memorial Lufkin, where she was treated for injuries described as non-life-threatening.

The driver of the Freightliner, Jose Quevedo, 43, of San Antonio, was not injured in the collision. No other vehicles were reported to be involved.

Authorities have stated that the investigation remains ongoing, and no additional details regarding contributing factors have been released at this time.

Investigation: Despite the preliminary rear-end collision report, the investigation into the crash circumstances continues. Typical investigations of passenger vehicle-truck highway intersection crashes focus on speed, following distance, and driver reaction time. Because the semi-truck was turning right, investigators will likely examine the functionality and visibility of the truck’s lights, the predictability of the turning maneuver, and whether traffic conditions provided following drivers with adequate notice.

The investigation remains active, and authorities have not announced whether citations will be issued.
